New Patient Forms

Washington Park Chiropractic now EMAILS patients a link to your forms in our patient portal so they can be completed before you arrive at our office. Please check your EMAIL or log into your patient portal (using the scheduling linke below) after scheduling to be sure you have access to your new patient intake form. New Patients should arrive 15 minutes early to allow for parking and getting comfortably settled at our office.
